PRIVACY POLICY
 

Pellow Family Chiropractic (PFC) values the relationship we have with our patients and recognises the importance of maintaining the confidentiality of the personal information supplied to us.  PFC is subject to the Privacy Act 1988 and the Privacy Amendment (Enhancing Privacy Protection) Act (2012) and is required to comply with the National Privacy Principles contained in that Act.

In relation to our patients, the personal information collected by PFC includes date of birth, current address, contact numbers, email address, nature of occupation, private health fund information, the name and contact details of the patient’s current GP, and the patient’s medical history and health complaints/concerns. This information is collected directly at the time an individual chooses to register for care at PFC and may be updated and added to over time.  PFC’s primary purpose in collecting personal information is to enable us to fulfil our service commitments by providing appropriate chiropractic care and patient education.

PFC only uses personal information to provide patients with services associated with being a patient of this practice. This may include but is not limited to:

    • charging and billing and the collection of debts
    • providing information on new products or services to be offered by Pellow Family Chiropractic
    • information gathering to improve services
    • sending out SMS reminders for upcoming appointments
    • keeping you informed about matters related to Pellow Family Chiropractic and the profession’s activities
    • provide information about chiropractic

PFC does not sell or share personal information without your direct and written permission.  With your permission, we will disclose personal information to a third party such as another chiropractic clinic (for example if you move away and would like your records transferred to a new provider of care) and or an insurance company in the event of a claim.  In very rare circumstances we may be required to share your personal information as required by law.

Where personal information is disclosed to third parties, PFC seeks to ensure that they comply with the requirements of the Privacy Act. However, PFC is not responsible for the misuse of personal information by these third parties.

PFC takes reasonable steps to protect your personal information from misuse, loss and from unauthorised access, modification or disclosure.  PFC imposes strict confidentiality rules on its employees who have access to the databases that store personal information and/or to the servers that host these services.

While PFC uses all reasonable endeavours to ensure that you provide personal information in a secure environment, no data transmission over the internet can be guaranteed as totally secure. An individual has the right to access and, if necessary, to correct personal information provided to and held by PFC.  The accuracy of the information PFC holds depends to a large extent on the information provided by patients.  Individuals who have provided personal information can assist in maintaining the accuracy of the information by notifying PFC of changes to their personal details.

PFC is committed to providing our patients with a fair and responsive system for handling and resolving complaints. You have a right to complain and to have your complaint handled efficiently. PFC believes that in receiving your complaint, PFC is provided with a valuable opportunity to improve the services PFC delivers to you and maintain your confidence in PFC services.

If at any time you wish to lodge a complaint in respect of the handling, use or disclosure of your personal information by PFC you may notify PFC of your complaint by completing the Complaints Form and submitting it by post to the Privacy Officer, Pellow Family Chiropractic, Shop 5/343 Mackenzie Street, Toowoomba, Qld, 4350. PFC will investigate and advise you of the outcome of a complaint within 21 days of receipt of the complaint.
